Home Care Routine After Microneedling: A Day-by-Day Guide

After microneedling, home care moves in three stages: the first 24 hours with only a gentle hydrating serum and no makeup, days 2 to 7 with a gentle cleanser, a plain moisturizer and mineral sunscreen, and then a slow return of actives from about day 7. Write it out day by day for your client, because the first week decides how well the skin recovers.

The first 24 to 48 hours

For the first night, your client should leave the skin alone as much as possible: no makeup, no cleanser until the next morning unless your protocol says otherwise, and only the hydrating or post-procedure serum you send home. A plain hyaluronic acid serum or a calming balm without fragrance is typical. Ask your client to sleep on a clean pillowcase, avoid touching the face and skip the gym, sauna, hot showers on the face and swimming pools for at least 48 hours to limit heat and bacteria.

Redness similar to a mild sunburn, tightness and slight swelling are expected for one to two days, and pinpoint marks may be visible. Tell your client this in advance so they do not reach for strong products to fix it. Make sure the aftercare sheet also says what is not expected: increasing pain, warmth that spreads, pus, blisters or fever. Any of those means contacting their physician or your practice's medical director promptly.

Days 2 to 7: gentle and protected

From the second day, the routine is short: a gentle, fragrance-free cleanser with lukewarm water, the hydrating serum, a plain moisturizer and a mineral broad-spectrum SPF 30 or higher every morning, reapplied when outdoors. Mineral filters such as zinc oxide are usually better tolerated on freshly treated skin. Light flaking around days 3 to 5 is normal; your client should let it shed on its own and never scrub or pick at it.

Hold all actives during this week. That means no retinol or prescription retinoid, if their physician prescribes one, no exfoliating acids, no vitamin C serum, no scrubs, no benzoyl peroxide and no at-home devices. Mineral makeup can usually return after 24 to 48 hours, applied with clean brushes. Long sun exposure should be avoided for the full week. Put each of these in writing with a date, since most clients forget verbal instructions by the next morning.

Bringing actives back and preparing the next session

Around day 7, once all redness and flaking have settled, actives can return one at a time. Start with vitamin C in the morning, then retinol or a gentle acid at night a few days later, at a reduced frequency. If the skin stings, step back for a few more days. Microneedling is usually done as a series of three to six sessions spaced four to six weeks apart, and home care between sessions supports the collagen work you are doing in the treatment room.

Pause retinol and exfoliating acids again three to five days before the next session, and ask your client to avoid tanning before it. At each visit, ask how the first week went and adjust the aftercare if needed. Note that the scope of microneedling for estheticians varies by state, including needle depth and whether medical oversight is required, so make sure your protocol and aftercare match your state's rules.

Worked example

Worked example: first microneedling session for texture

A client in her early 40s with normal to dry skin, Fitzpatrick type III, uneven texture and mild marks left by breakouts years ago. She uses retinol four nights a week and a vitamin C serum daily, and runs outdoors most mornings. She is booked for a series of three sessions, five weeks apart.

The plan

  1. 1Days 1 to 5 before the session: Retinol paused and morning runs moved indoors or kept short with sun protection.
  2. 2Day 1, after treatment: Hydrating serum only, no makeup, no workout and a clean pillowcase.
  3. 3Days 2 to 7: Gentle cleanser, hydrating serum, plain moisturizer and mineral SPF, no actives.
  4. 4Day 8 onward: Vitamin C back in the morning, then retinol two nights a week from day 10.
  5. 5Week 5: Second session, with the same pause and aftercare schedule repeated.

Home care

  • Fragrance-free cleanser with lukewarm water from day 2.
  • Hyaluronic acid serum twice daily during the first week.
  • Mineral SPF 30 or higher every morning, reapplied before any time outdoors.

The esthetician brought retinol back at two nights a week instead of four, because the client's skin was drier after treatment. She planned to return to the usual frequency after session two.

Questions professionals ask

When can my client wear makeup after microneedling?
Most protocols allow mineral makeup after 24 to 48 hours, applied with clean brushes. Follow your device and product instructions, and keep heavy foundations off until the skin has settled.
When can retinol be used again after microneedling?
Usually around 5 to 7 days after treatment, once redness and flaking have fully settled, starting at a lower frequency. Clients with sensitive skin may need a few extra days.
